Dear Patient：

Welcome to our hospital and we appreciate your trust and support. In order to carry out the medical services smoothly, and ensure you enjoying the quality services and protect the rights and benefits of both parties, we, hereby notify you of “Patient’s Rights and Obligations”. We hope that you would understand and cooperate with us, so that we can jointly create a warm and harmonious medical service environment. Wish you recover at the earliest.

一、您的权利和义务：    

Patient’s Rights and Obligations

（一） 在我院就诊中您享有的权利：

While enjoying our medical services, you are entitled to the below rights:

1、有权获得基本医疗保健，不因国籍、性别、年龄、种族、信仰或社会经济地位而受到歧视。

The rights to access to the basic medical care with no discrimination on nationality,sex, age, believing or social status.

2、有权在安全的医疗环境中接受诊疗护理服务，个人隐私得到妥善保护。

The rights to enjoy medical services in a safe environment. Personal privacy shall be properly protected.

3、有权参与诊疗过程的协商与讨论，并决定治疗方案，包括选择其它治疗办法或拒绝治疗，需要在相应的文件上签字后方可行使此权利，但您作出的选择不能违反国家的法律法规。

The rights to participate in the discussions during the process of medical services and decide the treatment plan, including selecting other ways of treatment or rejection of treatment, which shall be carried out after signing related documents. Nevertheless, the choice being made by the patient should not be against the laws, rules and regulations of the State.

4、有权询问并得知关于病情诊断、检查检验结果、治疗方法及预后情况。

The rights to inquire about the diagnosis of the sickness, the result of examination, treatment method and future effect.

5、有表达减轻疼痛的权利。

The rights to express reducing pain

6、有权要求医护人员提供与疾病护理、用药、饮食或生活等相关的健康教育资料。

The rights to ask the doctors and nurses to provide related health education materials concerning nursing, application of medicine, diet and so on.

7、有权要求财物保护、免受人身攻击。

The rights to ask for property protection and free from personal attack.

8、当您感到上述权利受侵犯，您可以向医务人员提出意见。若对就医过程有任何抱怨、医疗纠纷或建议时，有权向医院有关部门提出并得到回复。

If you feel that your above rights are infringed, you can talk to the medical personnel. If there are any complaints, medical disputes or suggestions during the process of medical services, you are entitled to bring up the issues to the concerned department which needs to give replies.

（二） 在我院就诊中您应履行的义务：    

While enjoying our medical services, you are required to follow the below obligations:

1、 请主动、正确告知医护人员患者的健康状况、婚育史、过去史、药物过敏史、目前是否罹患传染病等信息。

Properly inform the doctor of patient’s health status, marriage, history, medicine allergy, and also notify whether the patient is suffering from any infectious diseases.

2、 请听从导诊护士的指引，有秩序的候诊，不要随意翻阅其它就诊者的病历资料。若因个人原因不能按时就诊，系统将重新排序，请自觉遵守。

Please follow the guidance of the nurse for diagnosis in an orderly way. Don’t look over the medical records of other patients. If can’t see the doctor in time due to patient’s personal reasons, the computer system shall make the queuing list again, which has to be followed by the patient.

3、 请主动配合医院工作人员的工作，保证“一诊室一医生一患者”，进入诊室后，自觉关门，以保护您的个人隐私。

Please cooperate with the medical personnel and ensure “one diagnosis room with one doctor and one patient only”. After entering the diagnosis room, please close the door to protect your personal privacy.

4、请遵守医院的就医规定或治疗流程，勿要求医生提供不实的资料或诊断证明，遵守医院门禁、感染控制措施，不得在院内吸烟，不得在诊室、治疗室、候诊区和检查区大声吵闹，避免影响其他患者治疗或他人权益。

Please abide by the regulations or treatment process of the hospital. Never ask the doctor to give any false materials or diagnosis certification. Follow the rules of entrance and infection control measures. Don’t smoke within the hospital. Don’t make loud noises in the diagnosis room, treatment room, waiting area and examination area to avoid affecting other patient’s rights.

5、请配合医嘱进行治疗，珍惜医疗资源，妥善利用医院各项设施。

Please follow the doctor’s advice for the treatment. Treasure the medical resources and properly utilize the facilities of the hospital.

6、请尊重医务人员的劳动及人格尊严。

Please show respect to the medical personnel for their work and personal dignity.

7、请按时、按数缴纳医疗费用。

Please pay the actual medical fees on time.

二、医院的权利和义务：    

Hospital’s Rights and Obligations

（一） 我们有义务为您提供安全、舒适的医疗环境，有权利按照医院规定对您进行管理。

We have the obligations to provide you with a safe and comfortable medical environment, and we have the rights for the administration in accordance with the rules and regulations laid down by the hospital.

（二） 我们有权收取经物价部门核准的医疗服务费用，并向您提供费用清单和正规发票。

We have the rights to charge the medical services according to the prices verified by government price control department, and we will provide you with detailed expense list and official invoice.

（三） 如果您在本院被确诊国家规定必须向卫生防疫部门上报的传染病，医院有义务遵守国家规定，及时上报，并将依法律规定对您采取相应的诊疗措施，甚至限制您的某些人身自由。

If you are diagnosed with certain infectious disease which has to be reported to Sanitation Quarantine Department regulated by the State, the hospital is obliged to observe the rules and timely report the case to the superior authority, and shall take necessary medical measures, even restrict part of your freedom.

（四） 我们有权拒绝有违诊疗规范或医疗管理法律法规的医疗服务要求，但有义务采纳合理的建议和意见，改善服务质量，并承诺绝不因投诉影响医疗服务质量。并且对于易受伤害患者予以优先照顾和妥善保护。

We have the rights to reject the demand for any medical services violating laws, rules and regulations, but we are obliged to take the reasonable comments and suggestions to improve the services, and we commit the medical services shall not be affected due to complaints. We will take good care and properly protect the patients vulnerable.

（五） 本院为教学医院，有权让见习、实习医务人员在有资格医务人员指导下参与诊疗活动，但不允许这些人员独立进行诊疗活动。临床教学活动涉及门诊和住院的相关医疗过程中，我们将充分保护您的隐私。

The hospital is a teaching hospital, hence, we are entitled to have trainees and practice medical personnel to participate in the diagnosis and treatment activities under the qualified medical personnel, but they are not allowed to conduct such activities on their own. Clinical teaching activities might involve in the medical processes of outpatient and inpatient, but we will fully protect your privacy.
